RESIDENCY DOCUMENTS (Required in accordance with SC Policy JCA-I) - Four (4) documents required

The following documents must also be provided. Documents should be uploaded directly to the online registration form. If you are unable to do so, please contact your child's school.

1. Residency Verification Form (click to view and download)

2. Evidence of Residency (one of the following is required)
- If you own your home: record of recent mortgage statement and/or property tax bill
- If you rent: copy of lease and copy of recent rental payment or Section 8 Agreement
- If you live with a family member or friend: Landlord Affidavit (click to view and download)- must be notarized with both parties' signatures. In addition, the homeowner must also provide evidence of residency using one of the options above.

3. Evidence of Occupancy: recent bill dated within the past 60 days showing a Freetown or Lakeville address (one of the following is required)
- Gas bill
- Oil bill
- Electric bill
- Home telephone bill (not cell phone)
- Cable bill
- Excise tax bill

4. Evidence of Identification (Registrant) - must be presented in person to the school registrar, do not upload a copy (one of the following is required)
- Valid driver's license
- Valid MA photo ID card
